Member Rights & Responsibilities

As a member receiving services from Bookmark Health, you have the right to:
Dignity & Respect: To be treated as a valuable person and to have your privacy respected by all staff.
Freedom from Harm: To be safe from physical, verbal, sexual, or financial abuse, neglect, and exploitation.
Self-Determination: To make choices about your life, including which providers you work with and how your services are delivered.
Freedom of Movement: To be free from any form of restraint or seclusion used as a means of coercion, discipline, or retaliation.
Participation: To be an active part of your Individual Support Plan (ISP) meetings and to agree or disagree with the plan.
Communication: To receive information in a way you understand, including access to interpreter services at no cost to you.
Privacy: To have your personal and medical information kept confidential, only shared with those authorized by law or your consent.

To help us provide the best possible support, we ask that members and families:
Participate: Stay involved in the development of your support plan.
Communicate: Keep your Support Coordinator and Bookmark Health informed of any changes in your address, health, or needs.
Respect: Treat our Direct Support Professionals (DSPs) with the same dignity and respect you expect from them.
Collaborate: Work with your team to notice the quality of services and provide honest feedback.

If you are ever unsatisfied with your services, you have the right to be heard.
Contact Us First: We want to make it right. Please speak with our program manager at 602.345.1118.
Contact your Support Coordinator: If we cannot resolve your concern, your DDD Support Coordinator can help you request a change of provider.
Formal Grievance: You have the right to file a formal complaint with the Division of Developmental Disabilities.
